Application
Instructions and Selection Criteria
Please
read and follow these instructions carefully.
Failure to follow instructions will prevent your application from being
considered.
General
Information
Leadership
DCDS seeks a diverse cross-section of men and women from the
dental profession who are committed to the well-being of dentistry and
interested in assuming greater responsibility for future society leadership.
Candidates
who meet any of the following criteria are eligible to apply:
- members of the Dallas County Dental Society *
- members of a Component of the Texas Dental Association
- practicing dentists in the Dallas/Fort Worth Metroplex
Applications
for the Leadership DCDS program are accepted from March
1 through September 1 this year. Applicants are notified by September
1 of acceptance into the class that begins September 14. Since
class size is limited, applicants are encouraged to reapply if not selected.

Members of the DCDS will receive a discounted fee.
Application
Instructions
Complete
the four-page application form by using a typewriter, hand print in black
ink, or complete the form electronically from the Dallas County Dental
Society Web site. Hand printing must be clearly legible. Applications
may be rejected if they cannot be read by the Selection Committee or if
the application has not been completed properly using the following instructions:
- Limit answers to the available space on each page. Additional pages
will not be considered.
- Do
not include other attachments such as resumes. Additional attachments
will not be considered.
- Only
one completed recommendation form or letter will be accepted. This recommendation may not be from your employer.
- The required
photo may be a personal snapshot or passport photo. The photo must fit
in the designated space on the application form and not obscure any
application text. If necessary, trim your photo to the required size
(approximately 2 x 2 inches).
Note: Oversize
photos are not acceptable.
- Three
(3) copies (and the original) application must be included.
- Submit
all required application materials together in one envelope including
your letter of recommendation. You may enclose the recommendation letter
in a smaller sealed envelope inside the application envelope containing
the rest of the application materials.
- Your
completed application with all required signatures, recommendation letter
and photo is due at the Dallas County Dental Society, located at 13633
Omega Road, Dallas, TX, 75244, by close of business September
1 . (Please send Attn: Leadesrhip DCDS.)
- Your
application will be not be considered if the application is received
late, if it is not complete, if copies are not included, if the recommendation
form or letter is not enclosed, or if the photo is missing.
Application
From the Web Site and Electronic Completion by Computer
You
may obtain application and recommendation forms below.
You
can then use a computer to enter the information directly onto the form
by typing within a browser window. You will then print out, sign, include
attachments and mail to the Dallas County Dental Society, Attn: Leadership
DCDS, 13633 Omega Road, Dallas, TX 75244.
Note: You must have Adobe Acrobat in order to open and print
the forms. If you do not have this program, you can download a free
copy by clicking the Adobe download icon on the DCDS Web site.
Selection
Criteria
Leadership
DCDS participants are selected by members of the Leadership
DCDS Selection Committee, which is composed of society members.
Criteria
for selection includes:
- A history
of involvement or a strong desire to become actively involved in the
dental community
- An interest
in personal development and oral health issues
- A willingness
to serve the Dallas County Dental Society following completion of the
program by assuming greater leadership responsibilities within the society
- A commitment
to program attendance and participation requirements
- A commitment
to mentor the next generation of DCDS and dental leaders
Selection
is based solely on an applicant’s merit from the information provided
in the application.
Tuition
Tuition
for the program is $995. An applicant who is a current member in good
standing with the Dallas County Dental Society is eligible for a $245
discount. Tuition is due in full by September 14 after acceptance into the
program. Travel and lodging expenses for off site meetings will be paid
by the participant.
Fee
Payment Plan
Tuition
payment terms may be arranged. If approved, the fee will be divided into
3 equal payments with the first due by September 14 and the last by November
14. Please enclose a letter with your application requesting a fee payment
plan.
Attendance
Requirements
Applicants
must be able to fulfill program attendance requirements as follows:
- Attendance
is required at the first meeting in September. There are no exceptions
to this mandatory requirement. (Failure to attend is an automatic drop
from the class.)
- Attendance
is expected for each full day session during the months of September through
May. Sessions are usually held on the third Friday of each month from
9 a.m. to 4 p.m. A curriculum calendar is provided to each participant
upon acceptance into the class to allow planning for the year.
- Attendance
is required during the session at the TDA meeting. Participants are
required to pay their own travel and lodging expenses.
Participants
who miss two or more classes will not receive a certificate of completion.
